The Short Version
Today it’s the ending of The Silent Patient, by Alex Michaelides, and that twist.
The short version: Theo, the therapist telling us the story, is the man who broke into Alicia’s house the night Gabriel died. His story about his own marriage happened six years earlier. Let’s break it down.
The Twist
Here’s the line that changes everything. Theo has been following his wife’s lover, a man who meets Kathy in secret. One day he follows him home, and watches him eat dinner with his wife.
At the very end of part four, we learn that lover’s name. It’s Gabriel. And the wife Theo was watching through the window was Alicia.
The Timeline Trick
So how did we miss it? Theo’s chapters about Kathy are placed between his chapters at the Grove, so we assume they happen at the same time. They don’t. They happened six years earlier, in the summer of the murder.
Alicia’s diary is dated, and Theo’s marriage chapters never are. That’s the whole trick. Two stories, told side by side, about the same summer.
The Man Watching
Remember the man in Alicia’s diary? The one watching her from the park and across the pond, in a cap and sunglasses? That was Theo.
He was stalking the couple that broke up his marriage. And on the hottest night of the summer, he bought a mask and gloves and went into their house.
The Night Of The Murder
Theo held Alicia captive, then waited for Gabriel to come home. He cut the lights, knocked Gabriel out, and tied him to a chair.
Then he made Gabriel choose: you die, or she dies. And Gabriel said he didn’t want to die. Theo fired one shot into the ceiling, untied Alicia’s hands, and left.
Who Pulled The Trigger
So Alicia did kill Gabriel. She picked up the gun and shot him five times. Theo never touched him.
That’s why there was a bullet in the ceiling, and why only Alicia’s fingerprints were on the gun. Theo was wearing gloves.
Why She Stopped Talking
This is where the painting matters. In the Greek tragedy Alcestis, a husband lets his wife die in his place. She comes back from the dead, and she won’t speak.
When Gabriel chose his own life over hers, Alicia felt she had died. It echoed her father, who once said he wished she had died instead of her mother. And as Alicia writes, the dead don’t talk.
Why She Spoke To Theo
So why does she finally speak? Because she recognized him. He used the same phrase the intruder used, that he wanted to help her see clearly, and he smelled of cigarettes.
She told him a false version of the night to watch his reaction. Theo corrected her. He knew there was a shot in the ceiling, because he fired it.
What Theo Does Next
Theo can’t let her tell anyone. He injects her with morphine, and she falls into a coma she’s not expected to wake from. He frames another doctor, Christian, who had secretly treated Alicia years before.
Christian is arrested, the hospital closes, and Theo is offered a promotion. He’s gotten away with it.
The Diary
Until the last chapter. Alicia hid one final diary entry in the back of a painting, the one Theo said he didn’t like. In it, she wrote down everything, including who he is.
A detective arrives at Theo’s house and reads it aloud. Theo doesn’t run. He feels relieved.
The Last Scene
The book ends with snow, the snow everyone at the hospital kept waiting for. Theo catches a snowflake as it melts in his hand.
At the start of the book, Theo told us that as a boy he caught snowflakes, and that holding happiness is like holding snow. He ends in his abusive father’s old house, having become just as violent. And his whole book is his confession.
My Verdict On The Ending
I love how fair the twist is. Go back and the clues are everywhere. The snowflake ending gives it a real shape.
What didn’t work: Kathy never reacts to her lover’s murder, and Alicia’s six years of silence are hard to believe. Tell me in the comments: did the twist get you? My clues video and review are linked below.
